This guide covers all mandatory story puzzles in Resident Evil Requiem. Major story spoilers are included. Therefore, new players should proceed carefully.
This walkthrough focuses only on required progression puzzles. Optional item locks and minor detours are not included. However, every critical solution appears in clear steps so beginners and speedrunners can bookmark this page.
Grace – Fuse escape sequence
After waking in Room 203, run to the end and open the drawer to obtain the Cherub Key. Then unlock Room 201.
Inside, you will find a locked fuse box. You cannot open it yet. Return to the Nurse’s Station and use the lighter to reach the corpse blocked door.
When the giant female monster appears, push the shelf toward the tool cabinet and collect the screwdriver. Next, return to Room 201 and open the fuse box. Finally, install the fuse to unlock the barred exit.
Grace – Three quartz cube puzzle
After separating from Leon S. Kennedy, Grace must collect three Quartz pieces to unlock the white exit door. Inventory management becomes important here. Save frequently and plan item slots carefully.
Moon Quartz
Enter the Guard’s Office and read the Care Center Pamphlet. Go downstairs to meet Emily and collect the West Wing Keycard.
In the Chairman’s Office, shade the notebook with the pencil to reveal the clue. Input the sequence Moon, Sun, Star, Moon to receive the Moon Quartz.
Sun Quartz
Retrieve the Red Jewel from the Unicorn Trinket. Use it to unlock The Parlor and collect the East Wing Keycard.
Defeat or avoid the giant child monster after collecting Security Level 1 access. Then use Corrosive in the Lead Researcher’s Office puzzle and input Star, Sun, Moon, Sun to obtain the Sun Quartz.
Star Quartz
In the Garage, solve the Organ Transport Box puzzle in this exact order:
Rotate dial to Hand
Adjust right marker to middle
Switch 5 down
Switch 2 down
Rotate dial back to Auto
Collect Artificial Lungs. Then retrieve Artificial Heart from Cold Storage. Install both organs in the Examination Room corpse. Defeat the revived enemy to obtain Security Level 2 access.
Escort Emily to the Security Manager Office to secure Level 3 access. The story will shift to Leon afterward.
Grace – Underground joint plug puzzle
Collect three Joint Plugs from The Holding Cells, Furnace area, and Processing zone.
Use Square Socket Wrench to activate the Collection Pool mechanism. Drain water in Processing and survive the grinder sequence. Push enemies into the machine until it shuts down.
Finally, install all three Joint Plugs at the generator. Carry Emily to the elevator to complete this section.
Grace – Helicopter key puzzle
In the VIP Suite, rotate the wine label correctly to retrieve the Double Helix Model. Insert it into the chest mechanism.
Proceed through the action sequence to Collection & Testing. Retrieve the helicopter key and escape before the explosion.
Leon – Detonator assembly
Upon reaching Raccoon City, Leon must assemble three parts: Distributor, Signal Receiver, and Relay.
Retrieve Distributor from Logistic Warehouse rooftop.
For Signal Receiver, solve the dual battery puzzle in Underground Parking Garage. Install both batteries to unlock the exit. Repair the Signal Receiver with the Repair Kit from the BSAA container.
For Relay, collect Gas Can, refuel the generator, zipline to Willis Tower, and retrieve the final component.
Return to Central Camp and assemble all parts to destroy the gate.
Final decision – Hope or Destruction
During the final Elpis sequence, players must choose between Hope and Destruction.
Hope leads to the true ending and an additional boss fight.
Destruction results in a bad ending. However, players can reload the save to choose differently.
